Heart Function Clinic

Information on this page is meant for Physicians.

When a patient's heart can no longer meet the metabolic demands to function normally they suffer from congestive heart failure. There are many causes including coronary artery disease, cardiomyopathy, or valvular heart disease. These patients require intensive monitoring and management as an outpatient to prevent hospital admissions and help them function day to day.

The Heart Function Clinic at Halton Healthcare - Oakville Trafalgar Memorial Hospital is an outpatient program operating since 2000. It is staffed by the Oakville Cardiologists group and two nurses with advanced training in educating and caring for patients with heart failure.

The goals of the clinic are to optimize medical treatment, optimize quality of life, and reduce hospital admission for acute congestive heart failure.

The clinic provides:

  • Medical assessment and follow-up.
  • Management of medications and other modalities of heart failure treatment.
  • Patient and family education about diet, self-monitoring and assessment.
  • Continued support through regular clinic visits at the outpatient clinic and telephone monitoring.

Referrals to the Heart Function Clinic are made through the offices of the Oakville Cardiologists.
